Home Health Nurse Specialist

About the RoleWe are currently seeking experienced Private Duty Nurses LPN to join our team in providing high-quality care to patients in their homes.
The ideal candidate will have a passion for delivering compassionate and personalized care, with a strong commitment to patient safety and well-being.
Responsibilities- Implement physician-prescribed plans of care for patients
- Conduct thorough assessments and coordinate care with other healthcare professionals
- Administer medications, treatments, and therapies as prescribed by physicians
- Document all care provided in a timely and accurate manner
- Provide education and training to patients and their families on disease management and self-care techniques
To be successful in this role, you must have:
- An active, unencumbered LPN license in the state where you will practice
- Current CPR certification from American Red Cross or equivalent
- Six months prior hands-on nursing experience (preferred but not required)
- Reliable transportation
