- Care Coordination: Facilitate collaboration among primary care, behavioral health services, and other healthcare providers, ensuring seamless patient transitions.

Experience: A minimum of two years in case management within mental health settings, with a focus on community resources.
Education: A Master’s degree in Social Work, Psychology, Sociology, or a related field is required.
Licensure: Candidates must hold a valid California license as a Professional Clinical Counselor, Marriage and Family Therapist, or Clinical Social Worker.
