Job Description

At Project Transition, our mission is to empower individuals with serious mental illness, co-occurring substance use disorder, and/or dual diagnoses of SMI and IDD to live a meaningful life in the community on their own terms.

This position plays a vital role in creating awareness, motivation, and tools necessary for individuals to develop and maintain a well-rounded, healthy lifestyle.

The Wellness Specialist will conduct initial assessments of each member's health and monitor their weight and general measurements through ongoing evaluations.

Collaborating with members and the treatment team, the Wellness Specialist will create individualized Wellness goals tied to their treatment plans and provide tools and resources to support goal attainment.

Key Responsibilities:

  • Provides individualized health coaching to members to support achieving their health objectives.
  • Teaches others about managing their mental and physical health.
  • Supports members in learning healthy choices related to health and wellness.
  • Creates and adapts wellness plans to overcome barriers to good mental and physical health based on member needs.
  • Facilitates groups to support improved health outcomes for members.
  • Works closely with the treatment team to engage members in appropriate services and resources.
  • Assists members and the team with coordination of care.
  • Promotes engagement with primary care and health resources in the community.
  • Completes all documentation on day services provided.
  • Completes all tracking and reporting requirements for outcomes and evaluation.
  • Supports the development of healthy menus and meal planning.

Performance Expectations:

  • Participate in multidisciplinary treatment teams and support and implement interventions and directives as directed by the team.
  • Demonstrate compassion and concern when supporting members, embracing Project Transition's Mission and Core Values.
  • Approach member engagement from a non-judgmental stance, understanding that member behavior is driven by experience, which may include trauma.
  • Treat and speak to members with supportive kindness, even when members demonstrate intense behavioral or emotional actions.
  • Seek out appropriate support, consultation with clinicians or psychiatrists, in conjunction with the Program Director/Manager or obtain supervision when uncertain about how to respond or support a member effectively.
  • Report back to the treatment team any observations of member behavior that suggests a member may need additional treatment interventions and/or support.
  • Engage with all external parties/individuals with professionalism and a positive customer service approach, understanding that they are always representing the organization.

Qualifications:

  • Minimum of a High School Diploma and/or associate degree with Wellness Certification. Bachelor's degree in a health-related field preferred.
  • Ability to have fun in the workplace.
  • Current PA Driver's License and maintain a clean driving record.
  • Two (2) years of experience in a drug and alcohol setting.
  • Skilled in Microsoft Office.
  • High energy individual with a strong work ethic and ability to multi-task.
  • Must be a self-motivator who is able to work in a self-directed manner.
  • Ability to maintain confidentiality.
