Hospice Case Manager

The Hospice Case Manager at CareerNation provides professional nursing care to patients and their families via direct and/or telephone contact, following established standards and practices. This role involves coordinating with other members of the care team to ensure seamless care delivery, maximal coordination of efforts, and active patient participation in planning and care. Key responsibilities include:
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
- Follow established policies and procedures
- Maintain continuity of patient care in a home care setting
- Provide direction to licensed practical nurses, clerical assistants, medical assistants, and other non-licensed personnel
Requirements
To be successful in this role, you will need to have:
- Graduate of an accredited school of nursing
- Registered nurse licensed to practice in Texas (TX) with current TX registration and in good standing
- Bachelor of Science in Nursing (BSN) degree
- At least two years of recent pertinent professional nursing experience in a hospital, medical office, or other clinical setting
Work Schedule
This is a part-time, per diem, or full-time position with a competitive hourly rate of $00 - $52 per hour.
