At CareerNation, we are seeking a skilled Hospice RN Case Manager to join our team. As a key member of our care team, you will provide professional nursing care to patients and their families, ensuring seamless care delivery and maximal coordination of efforts.



Responsibilities:


  • Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures
  • Maintain continuity of patient care in a home care setting
  • Flushing IV catheter
  • Administering pain medication
  • Turning patient every 2 hours


Requirements:


  • Graduate of an accredited school of nursing
  • Registered nurse licensed to practice in Texas (TX) with current TX registration and in good standing
  • Record of prior stable employment and/or favorable references


Preferred Qualifications:


  • Minimum of one year experience as a registered nurse or two years as a licensed practical nurse in a pertinent practice area
  • Bachelor of Science in Nursing (B.S.N.)
  • Current BLS certification
  • At least two years of recent pertinent professional nursing experience in a hospital, medical office, or other clinical setting


Work Schedule:

3-4 days a week, Flexible Schedule



Pay:

$52 per hour
