Key Responsibilities- Health Monitoring and Support: Monitor the health conditions of individuals served and ensure quality services are provided.
- Medication Administration: Administer medications and provide documentation, as well as oversee and monitor staff members who administer medications.
- Medical Treatment and Personal Care: Provide prescribed medical treatment by physician's orders and personal care services.
- Assessments and Care Planning: Conduct assessments, implement healthcare services to be included in each Individual Service Plan (ISP), and respond to changes of condition.
- Staff Training and Development: Provide training to staff members on universal precautions, infection control, and medication administration.
- Liaison and Advocacy: Serve as a liaison and advocate with other medical and professional service providers.
- Education and Support: Provide training for individuals served on topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
- Education and Experience: Associate's Degree in Nursing or related field, or a graduate of an accredited LPN program. One year of experience working with the population served in a nursing capacity.
- Licensure and Certification: Current state LPN or LVN License. Additional certifications as required by state licensing regulations, such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
- Communication and Interpersonal Skills: Ability to effectively communicate with staff, individuals served, and families.
- Competitive Compensation and Benefits: Monday - Friday, daytime hours, with a competitive hourly rate of $26.49. Full compensation and benefits package for employees working 32+ hours/week.
