New Horizon Counseling Center serves a diverse population throughout Queens, Brooklyn, and Nassau/Suffolk. We address multiple comorbidities, such as chronic medical conditions complicated by substance use and mental health disorders.

Our clients face significant barriers to accessing behavioral and medical services due to social and economic factors. By addressing these challenges, we empower individuals to remain engaged in their care, reducing preventable hospitalizations and emergency care.

Key Responsibilities
  • Work collaboratively with shelter staff to engage clients in services
  • Help clients attain housing and personal goals
  • Develop and implement policies and procedures for nursing services
  • Supervise LPNs and manage medication administration
