Job Summary:

The RN Care Coordinator plays a critical role in overseeing the progression of care and discharge planning for patients requiring specialized services. This position emphasizes care coordination, communication, and collaboration with various stakeholders to ensure optimal outcomes.

Key Responsibilities:

  • Oversee the progression of care and discharge planning for identified patients requiring specialized services.
  • Collaborate with utilization management, nursing, physicians, and ancillary departments to progress care toward optimal outcomes.
  • Advocate for patients and families by identifying and addressing cultural, language, and socioeconomic barriers to care transitions.
  • Enhance the patient experience by promoting quality of care and cost-effective outcomes.

Requirements:

  • Graduate of an accredited school of nursing.
  • Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
  • California RN license.
  • BLS required within 3 months of hiring.

Preferred Qualifications:

  • Bachelor's Degree in Nursing (BSN) or related healthcare field.
  • At least five (5) years of nursing experience.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification.
