Nurse Care Coordinator

The Registered Nurse - Hospice Care provides high-quality patient care and coordinates services to meet the needs of individuals and families within their homes and communities. This role requires a strong emphasis on community health education and experience.
Key Responsibilities
Completes comprehensive assessments of patients and families to determine hospice needs.
Provides professional nursing care by utilizing the nursing process.
Assesses and evaluates patient status by writing and initiating plans of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
Requirements
Graduate of a National League of Nursing accredited school of nursing.
Current licensure in the state and CPR certification.
Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
Work Environment
The Registered Nurse - Hospice Care will work in a fast-paced environment, providing care to patients in their homes and communities. This role requires excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist. The ability to lift, position, and/or transfer patients, as well as lift supplies and equipment, is also required. A valid driver's license and automobile insurance are necessary for this role.
