About the Role

We are seeking a highly skilled and dedicated Patient Care Coordinator to join our team at Community Health Network. As a Patient Care Coordinator, you will play a critical role in ensuring the delivery of exceptional patient care and services.

Key Responsibilities
  • Provide daily leadership and support to the operations of the Medical A Unit
  • Exhibit excellence in clinical patient care, customer service, critical thinking, schedule management, relationship competencies, and organizational skills
  • Have the authority and autonomy to ensure the appropriate clinical and customer satisfaction outcomes are met
  • Effectively lead and manage a team to achieve exceptional patient care and services
Requirements
  • Graduate of a National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC)
  • 2+ years experience as an RN within specialty area or related specialty area preferred
