About the Role

We are seeking a highly skilled and experienced Senior Manager to lead our High Risk Care Field Operations team. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our high risk care programs, ensuring that our patients receive the highest quality care and service.

Key Responsibilities
  • Lead the management of the day-to-day operations of our high risk care programs, including virtual care teams and in-market/in-home primary care house calls providers.
  • Design and implement standard work processes, partnering with other High Risk Care and company leaders to determine operational strategies and evaluate team results and objectives.
  • Continuously improve our operations through Lean principles and Active Daily Management, ensuring that our patients receive the appropriate outreach, Health Coach support, visit scheduling, and care coordination during their enrollment and participation in our High Risk Care programs.
  • Build an engaged team, leveraging CICARE and other frameworks to meet patient care and team experience goals.
  • Actively communicate and partner with market and in-office care team leaders to ensure clear visibility of the High Risk Field Operations team performance and effective handoffs for patient care.
  • Travel to in-market locations of the High Risk Care field operations on a bi-monthly to quarterly basis.
Requirements
  • 3+ years of leadership experience within programs serving high risk patient populations, including care through a home-based care model.
  • 5+ years of experience in operations and process management.
  • 3+ years of experience in direct people management, with virtual team management experience preferred.
  • Deep knowledge of value-based care operational context, high risk care services, and home-based primary care/house calls operational models.
  • Proven ability to drive results and support change management and continuous improvement.
  • Proven ability to foster strong, collaborative peer relationships or team dynamics that ensure a supportive and engaged team culture.
  • Experience developing talent through mentorship and coaching, consistent feedback, goal setting, and accountability.
  • Proficient in the use of data management and analytics tools (Excel, Google Sheets, Tableau, etc.) to create and maintain program analysis and evaluation.
  • Proficient in and experience working with scheduling and office operations and communications tools (CareLink, Google Workspace, Slack).
  • Strong written and verbal communication skills.
  • Reside in and lead the operations of the team from the Eastern time zone within commuting distance of a One Medical practice/clinic location.
