Job Title: Advanced Practice Provider - Nurse Practitioner or Physician Assistant

We are seeking a highly skilled Advanced Practice Provider to join our team at LiveWell Choice, LLC. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our members in nursing homes.

Key Responsibilities:
  • Provide collaborative primary care and services to a panel of Longevity Health members, including treating in the most appropriate setting based on the member's goals of care.
  • Execute and deliver Longevity Health Model of Care activities, treating in the most appropriate setting, and develop and implement advanced care planning.
  • Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, Medication Reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, and educate care teams on member needs and early identification of changes in condition.
  • Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members on members' care needs, goals of care, treatment plans/changes.
Requirements:
  • State unrestricted licensed Advanced Practice Nurse or Physician's Assistant with National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant.
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Must be eligible to participate in the Medicare program as a provider and registered.
  • Valid state driver's license with a good driving record and proof of automobile insurance required.
Preferred Qualifications:
  • Minimum of 6 months experience as a Nurse Practitioner or Physician's Assistant.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/Technology proficiency required.
  • The ability to engage and communicate effectively with physicians, patients, family members - excellent people skills.
