Part-Time Hospice RN? We are looking for compassionate, dynamic, and organized professionals with hospice experience to join our

Hospice Family

serving clients in the surrounding areas.

As a registered nurse, you will recognize the importance of hospice care.

Your role will involve providing emotional support, ensuring patients can spend their final days with dignity and comfort.

Our

Hospice RNs

will conduct essential supervisory visits to observe the performance of assigned team members.

Engage in interdisciplinary team meetings, providing insights for the development, review, and modification of patient and family care plans.

Current On-Call Hours: 5 PM - 8 AM - On-Call Responsibilities Required with a Rotating Schedule.

Key Responsibilities:
- Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
- Implement and adjust individualized care plans as necessary.
- Ensure compliance with hospice policies and Medicare regulations.
- Educate patients and families on disease processes, self-care techniques, and end-of-life care.
- Supervise paraprofessionals delivering patient services.
- Prepare clinical documentation and participate in interdisciplinary group conferences.
- Contribute to hospice performance improvement initiatives.
- Maintain high levels of satisfaction among clients, families, and caregivers.
- Communicate effectively using current processes and technologies within the organization.

Qualifications:
- Current Michigan Nursing License in good standing.
- Minimum of one year of hospice experience.
- Empathetic approach towards patients and caregivers.
- Willingness to participate in field visits and office meetings as required.
- Ability to adapt to patients' needs on a full-time on-call basis.
