Position Summary
As a skilled Registered Nurse or Licensed Practical Nurse, you possess strong clinical expertise and a commitment to building meaningful relationships with both colleagues and patients. Your Role
In the capacity of a Care Coordinator, you will oversee the nursing care provided to patients during your shift. You will adhere to the Comprehensive Care Plan and assist in maintaining accurate documentation in patient records as necessary. Additionally, you will supervise the direct care staff on your shift, ensuring high standards of care are met.

Qualifications
General Requirements:
  • Current licensure as a professional nurse in New York State.

Knowledge and Skills:
  • Understanding of nursing policies and protocols.
  • Knowledge of medication administration, potential side effects, and relevant regulations.
  • Comprehensive understanding of nursing practices and patient needs.
  • Awareness of the organizational structure and responsibilities of team members.
