Job Summary

AdvisaCare is seeking a compassionate and skilled Registered Nurse to join our Hospice team. As a Hospice RN, you will provide emotional support and care to patients and their families, helping them navigate the end-of-life journey with dignity and comfort.

Key Responsibilities
  • Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs
  • Implement individualized plans of care and make revisions as needed
  • Ensure compliance with AdvisaCare Hospice policies and procedures, as well as Medicare regulations
  • Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns
  • Supervise paraprofessionals providing services to patients
  • Prepare clinical and progress notes, participate in interdisciplinary group conferences
  • Participate in the Hospice performance improvement program
  • Maintain high levels of customer satisfaction with clients, families, and caregivers
  • Communicate information using current processes and technology within the organization
Requirements
  • Current Michigan license in good standing
  • Minimum 1 year of hospice experience
  • Empathetic approach to patients and caregivers
  • Ability to adapt to patients' status and needs on a full-time on-call basis
