Key Responsibilities:
  • Assist and monitor daily living activities to promote independence and competency.
  • Encourage decision-making and quality of life through empowerment, safety awareness, and respect for dignity.
  • Monitor overall health and well-being, and assist with medication treatment and administration as needed.
  • Provide crisis intervention and emergency services when necessary.
  • Support individuals in maintaining and enhancing their social support network through community activities and relationships.
  • Plan and provide opportunities for community engagement and socialization.
  • Implement strategies to meet ISP/PSTP goals and support individual empowerment.
Requirements:
  • High School Diploma or GED required.
  • Minimum of six months experience in human services preferred.
  • Training provided by BHN.
  • Valid driver's license and reliable car for transporting individuals in the community.
  • Commitment to individual empowerment, growth, human rights, and community integration.
  • Familiarity with internet, word processing, and email systems.
