Intensive Case Manager

Project Renewal is seeking a highly skilled and compassionate Intensive Case Manager to join our team. As an Intensive Case Manager, you will play a critical role in providing support and guidance to individuals with serious and persistent mental illness and co-occurring substance abuse disorders.
Job Summary:The Intensive Case Manager will be responsible for assessing clients to determine their needs and developing individualized case management plans to address these needs. This will involve working closely with clients to identify their strengths and challenges, and developing strategies to help them achieve their goals.
Key Responsibilities:- Conduct comprehensive assessments of clients to identify their needs and develop individualized case management plans.
- Work closely with clients to identify their strengths and challenges, and develop strategies to help them achieve their goals.
- Provide support and guidance to clients to help them manage their daily lives, including accessing entitlements and benefits, and developing independent living skills.
- Monitor client progress and make adjustments to case management plans as needed.
- Collaborate with other service providers to ensure that clients receive comprehensive and coordinated care.
- Bachelor's degree with 4 years' experience working with mentally ill and forensic consumers.
- MSW and 2 years' experience working with mentally ill and forensic consumers.
- Valid New York State Driver's License required.
