The Housing Opportunities for People with AIDS (HOPWA) Case Manager is a critical role at Iris House, responsible for providing intensive case management and crisis intervention services to individuals affected by HIV/AIDS. This position requires a high level of professionalism, cultural competence, and a passion for delivering exceptional client services.
Key Responsibilities:
- Maintain a caseload of up to 25 clients, providing regular home visits and ensuring compliance with contract services.
- Develop and implement individualized service plans, incorporating motivational interviewing and evidence-based practices to engage clients.
- Monitor client progress, provide support and guidance, and assist with housing maintenance and medical appointments.
- Collaborate with other agencies and community resources to refer clients for treatment and services, ensuring seamless integration with Iris House programs.
- Participate in team meetings, case conferences, and supervision to ensure effective communication and teamwork.
- Stay up-to-date with the latest developments in case management and HIV/AIDS services, applying this knowledge to improve client outcomes and program effectiveness.
Requirements:
- Strong case management and crisis intervention skills.
