Griffin Hospital is seeking a highly skilled and compassionate Outpatient Nurse Navigator to join our team. As a key member of our Outpatient Care Coordination department, you will play a vital role in supporting the clinical, business, and program-related work of our Accountable Care Organization (ACO) staff.

**Key Responsibilities:**

  • Collaborate with ACO provider offices, inpatient teams, and other clinical and non-clinical staff across multiple hospital departments to ensure seamless patient care.
  • Provide expert navigation and support to patients, practitioners, and hospital employees to facilitate effective care management and readmission prevention initiatives.
  • Perform complex administrative duties, including identifying and coordinating available resources for beneficiaries.
  • Function as a key contact for ACO activities, ensuring timely and effective communication with stakeholders.

**Requirements:**

  • LPN or RN License in CT required.
  • Minimum 3 years of nursing experience in an acute care, post-acute, or home health capacity.
  • Demonstrated knowledge of regulatory guidelines.
  • Proficient in identifying measurable functional rehab goals.

**Preferred Qualifications:**

  • Excellent interpersonal and communication skills.
  • Ability to work collaboratively as a team member.
  • Proficient in using Microsoft Excel and able to learn quickly to use new computer software programs.

**Physical Demands/Working Conditions:**

  • Ability to travel frequently to hospitals, skilled facilities, patient homes, and other sites where patients receive care as indicated by patient need.
