Position Overview:
As a vital member of the healthcare team, the Clinical Nurse Specialist delivers comprehensive care management within the outpatient healthcare environment, adhering to established protocols, policies, and standards of the Cherokee Nation.

This role is integral to the collaborative care team, which may consist of healthcare providers, licensed practical nurses, medical assistants, and the patient cohort.


Key Responsibilities:


The primary responsibilities include delivering healthcare services to individuals qualified for assistance, in line with the self-governance agreement between the Cherokee Nation and federal authorities. Responsibilities encompass:

  • Conducting pre-visit preparations for patients scheduled for appointments.
  • Effectively communicating patient needs and proposed treatments to the healthcare team.
  • Maintaining and updating patient panels for the care team.
  • Identifying and managing high-risk patient populations for enhanced care coordination.
  • Monitoring performance metrics on a monthly basis and sharing insights with the team and patients.
  • Engaging in continuous quality improvement initiatives utilizing established improvement models.
  • Following up with patients regarding diagnostic tests, referrals, and appointments as necessary.
  • Coordinating nursing visits for patient follow-up.
  • Tracking and managing referrals efficiently.
  • Providing education and support for patient self-management.
  • Triaging patients who reach out for assistance and directing them appropriately.
  • Documenting all observations, assessments, interventions, treatments, and referrals in accordance with Cherokee Nation documentation standards.
  • Offering direct nursing care as needed by the team.
  • Submitting copies of all relevant licenses and certifications to Human Resources upon receipt.

Supervisory Role:
This position does not include supervisory responsibilities.

Qualifications:

Educational Background:
Must possess an Associate's degree in Nursing from an accredited two-year college or technical institution; no alternatives accepted.

Experience:
No additional experience is required.

Technical Skills:
Proficiency in database management, spreadsheet applications, and word processing software is essential.

Licenses and Certifications:


Must hold a valid Registered Nurse (RN) license from the Oklahoma Board of Nursing or a multistate RN license recognized by the OBN, with no substitutions allowed. Continuous maintenance of the license is required throughout employment. Certification in Basic Life Support (BLS) is mandatory and must be maintained. Advanced Cardiac Life Support (ACLS) certification is also required, to be obtained within six months of employment if not already certified.


Additional Requirements:


The candidate must not be under any sanctions by the U.S. Department of Health and Human Services or listed on any exclusion lists. Compliance with HIPAA regulations and maintenance of confidentiality regarding client information is essential.


Physical Requirements:


This role involves frequent standing, walking, sitting, and using hands for various tasks. Occasionally, the employee may need to lift or move items weighing up to 50 pounds.


Work Environment:


While performing job duties, the employee may encounter fumes or airborne particles and must be prepared for a moderate noise level in the workplace.
