Job Summary:

The Clinical Logistics Nurse will collaborate closely with Medical Directors to engage and educate the clinician team, optimize performance on defined key metrics, and ensure collaboration with the broader site care team. This position does not involve providing direct patient care. Success in this role will be demonstrated by tangible improvements in patient outcomes and care experiences, along with more efficient resource utilization.

Key Responsibilities:
  • Develop and implement site-specific workflows to achieve program goals and performance targets, aligning with hospital and Core priorities.
  • Partner with the Medical Director to implement Core's clinical processes and lead process optimization.
  • Provide timely status updates to Core leadership of implementation, barrier resolution, and action plans.
  • Understand, clearly articulate, and promote the adoption of hospital and Core clinical performance objectives with the physician/APP team.
  • Engage and educate clinician team(s) related to clinical performance initiatives, goals, and measurement.
  • Oversee and facilitate clinical performance activities.
  • Identify cohorts of patients eligible for specific patient-centric care initiatives.
  • Participate in daily workflows by collaborating with various care team members.
  • Enhance communication throughout the care team as the patient moves through their hospital stay.
  • Facilitate coordination between the hospital medicine team and hospital departments to ensure the delivery of workflows related to throughput, discharge facilitation, and transitions of care.
  • Collaborate with the Medical Director, DCO, DOO, and hospital leaders to share performance data with the physician team to drive continuous improvement.
  • Continually evaluate processes, tools, and workflows to identify opportunities for improvement.
  • Participate in hospital committees and special projects as appropriate.
  • Assist with orientation of new physicians and APPs to Core's processes, quality, and patient experience initiatives.
  • Collaborate with Medical Director, DOO, and DCO to contribute to team meeting agendas.
  • Serve as a point of contact for hospital staff (e.g., issues related to clinical processes and triage for other potential issues related to hospital medicine team).
Requirements:
  • Current RN licensure in good standing, in the state applicable to the position.
  • Strong clinical knowledge and experience in the Acute Care Hospital setting (Critical Care, In-patient Medicine, Emergency Department, Quality, and/or Case Management experience is helpful).
  • Ability to assess patient's clinical status and readiness for discharge.
  • Ability to work both independently and collaboratively.
  • Excellent organizational skills with the ability to multi-task and prioritize workload in a challenging environment while maintaining mindfulness and accuracy.
  • Excellent written and oral communication skills, including strong presentation skills.
  • Client service oriented with the ability to interact and communicate with individuals at all levels (e.g., patients, physicians, hospital staff, hospital administration and other colleagues).
  • Strong people skills to manage sensitive situations and confidential information. Position continually requires demonstrated poise, tact, diplomacy, and good judgment.
  • Proficiency with technology, including electronic medical systems and Microsoft Office Suite.
  • Exhibit growth mindset and team-orientated behaviors.
Education:
  • RN required.
  • MSN preferred.
Experience:
  • Minimum: 5 years nursing experience, with at least 2 years recently in an inpatient acute care or hospital setting.
  • Proficient with electronic medical systems.
  • 5 or more years inpatient nursing experience and 1-2 experience in a nursing leadership role, Case Management, Quality, Transitional Care, and/or post-acute services preferred.
  • Knowledge of process improvement methodology. Lean Six Sigma Green Belt (LSSGB) certification or equivalent, preferred.
