Job Summary

The Department of Veteran Affairs is seeking a highly skilled Primary Care Physician to join our team at the VA Illiana Health Care System. As a Primary Care Physician, you will be responsible for providing comprehensive medical care to a panel of patients managed under PCMM guidelines.

Key Responsibilities

  • Manage Chronic Conditions: Evaluate and treat patients with chronic conditions, such as diabetes, hypertension, and heart disease.
  • Collaborate with Care Teams: Work with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, and continuous care to assigned patients.
  • Preventive Care: Perform disease prevention-related activities, including gender and age-specific screenings, mental health screenings, and health promotion activities.
  • Mental Health Support: Screen for, evaluate, and treat uncomplicated common mental health disorders, including depression and common addictions, with referrals made to mental health providers as needed.
  • Supervise and Collaborate: Supervise or collaborate with primary care mid-level providers and participate in peer review of other primary care providers through periodic chart reviews.
  • Act as Surrogate: Act as a surrogate for absent primary care providers, including reviewing CPRS view alerts.
  • Attend Meetings: Regularly attend primary care staff meetings, departmental meetings, and medical staff meetings.
  • Timely Documentation: Ensure timely documentation of patient notes and encounter forms in accordance with facility policy.

Work Schedule: Monday-Friday, 7:45 a.m. - 4:15 p.m.
