Role Summary:
The Systems of Care (SOC) Coordinator is responsible for delivering a holistic approach to support services for at-risk youth and their families, as identified by various stakeholders including families and educational institutions.
Key Responsibilities:
- Implement and adhere to the System of Care model.
- Collaborate with a multidisciplinary team including the family, support systems, educational institutions, and community organizations.
- Employ culturally sensitive methods when engaging with diverse populations.
- Utilize evidence-based practices tailored to the unique strengths and needs of each youth and family.
- Facilitate communication and coordination among schools and community partners.
- Manage referrals and ensure the completion of necessary intake documentation.
- Document progress and maintain accurate records within the Electronic Health Record (EHR) system, following all relevant guidelines.
- Assess family needs across various life domains and assist in addressing them.
- Work with families to create and regularly review service plans that are family-driven.
- Initiate and maintain communication among families, identified supports, and involved agencies.
- Coordinate the implementation of service plans across all agencies serving children and families.
- Provide case management services and facilitate access to community resources.
- Conduct outreach, training, and marketing to promote SOC programming.
- Regularly evaluate client progress towards goals and make necessary adjustments.
- Respond compassionately and effectively to crisis situations.
- Deliver services in various settings, including home, community, and office environments, to best meet the needs of youth and families.
- Perform additional job-related duties as assigned.
Required Skills and Qualifications:
- Understanding of mental health services with a commitment to ongoing professional development.
- Compliance with HIPAA regulations and confidentiality standards.
- Ability to work independently while seeking supervision when necessary.
- Effective time management and organizational skills.
- Strong communication skills and the ability to build effective relationships with clients and colleagues.
- Critical thinking and problem-solving abilities.
- Proficiency in relevant computer applications and technology.
- Ability to meet deadlines and adhere to legal and ethical standards.
- Flexibility to provide services in local communities as needed.
- Valid driver's license and proof of insurance may be required.
- Availability for occasional evening and weekend hours.
Education and Experience:
Minimum requirement of a high school diploma or equivalent; an associate degree is preferred. A minimum of one year of experience working with children in a relevant setting is preferred.
Physical Requirements:
This role requires mobility within the office and travel to various locations within the designated service area. The ability to sit at a desk and work on a computer for extended periods is necessary, as well as the capacity to lift up to 15 pounds occasionally.
