Case Manager

Washington County Mental Health Services is seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support services to adults in mental health services, helping them to achieve their goals and improve their overall well-being.
Job Summary:We are looking for a collaborative, energetic, and team-oriented individual to provide a range of services to meet the support needs of adults in mental health services. This is a full-time benefited position primarily based in the community.
Key Responsibilities:- Provide support services to clients, including outreach visits, support services, accessing benefits, and assistance with independent living skills.
- Develop and deliver services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial, and other services.
- Assist clients in developing fulfilling lives by collaborating with them in implementing their treatment goals in the community.
- Assist in developing natural support networks for adults in accordance with their treatment goals.
- Collaborate and consult with clients, case managers, clinicians, community partners, hospitals, and others involved with the treatment team on an ongoing basis.
- Attend required meetings and trainings, including individual and/or group supervision and other agency committee work.
- Work collaboratively with other agencies providing services to clients in accordance with treatment goals.
- Attend staff meetings at WCMH as required.
- Other duties as assigned by supervisor.
- Bachelor's Degree in social work, human services, or related field.
- One year of services delivery with adults and children.
- Sensitivity to the unique needs of clients with a history of trauma.
- Possession of a valid driver's license, excellent driving record, and access to a safe, reliable, insured vehicle.
