Job Summary

We are seeking a skilled Case Manager to join our team at Washington County Mental Health Services. As a Case Manager, you will provide support services to adults in mental health services, including outreach visits, support services, and assistance with accessing benefits and independent living skills.

Key Responsibilities
  • Provide support services to clients, including outreach visits, support services, and assistance with accessing benefits and independent living skills.
  • Develop and deliver services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial, and other services.
  • Assist clients in developing fulfilling lives by collaborating with them in implementing their treatment goals in the community.
  • Assist in developing natural support networks for adults in accordance with their treatment goals.
  • Collaborate and consult with clients, case managers, clinicians, community partners, hospitals, and others involved with the treatment team on an ongoing basis.
  • Attend required meetings and trainings, including individual and/or group supervision and other agency committee work.
  • Work collaboratively with other agencies providing services to clients in accordance with treatment goals.
  • Attend staff meetings at WCMH as required.
  • Other duties as assigned by supervisor.
Requirements
  • Bachelor's Degree in social work, human services, or related field.
  • One year of services delivery with adults and children.
  • Sensitivity to the unique needs of clients with a history of trauma.
  • Possession of a valid driver's license, excellent driving record, and access to a safe, reliable, insured vehicle.
