The Case Management Director at ReqOverflow is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for their assigned area of responsibility in alignment with the goals of the Case Management Department.
Key Responsibilities- Oversee the overall day-to-day management of the case management program, including coordination, supervision, and administrative oversight of the case management team.
- Function as an expert clinical practitioner, case management subject-matter expert, resource, advisor, and leader for the members of the case management team.
- Supervise and monitor professional and support staff to ensure effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines.
- Monitor the quality of interactions, documentation, and assessments of the case management staff.
- Coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
- 2-year / Associates Degree in Nursing
- 4-year / Bachelors Degree in Nursing (2-year grace period) OR Masters Degree in Social Work
- 3+ years experience in Acute-Care Case Management or Social Work
- 2+ years supervisory experience
