Job Title: Residential Care Specialist

Join the Detroit Wayne Integrated Health Network team as a Residential Care Specialist, where you will play a vital role in providing high-quality residential services to individuals with serious mental illness, intellectual developmental disabilities, and co-occurring substance use disorders.

Job Summary

The Residential Care Specialist is responsible for assessing and placing members into the most appropriate residential settings, ensuring they receive the necessary support and care to achieve their goals. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively with diverse populations.

Key Responsibilities
  • Conduct comprehensive assessments of members to determine their residential needs and develop individualized care plans.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and service delivery.
  • Provide clinical guidance and support to Residential Care Coordinators and other team members.
  • Supervise the review and authorization of residential services and programs, ensuring compliance with regulatory requirements.
  • Monitor and evaluate the effectiveness of residential services, making recommendations for improvement.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Participate in staff meetings, trainings, and other meetings as required.
Requirements
  • Master's degree in Social Work, Psychology, Counseling, Nursing, or a related field.
  • Five years of professional experience in behavioral healthcare or a community mental health setting.
  • Valid State of Michigan clinical licensure (RN, LMSW, LMHC, LPC, LLP, or PhD).
  • Valid State of Michigan Driver's License with a safe and acceptable driving record.
Working Conditions

This role requires frequent travel to different sites throughout Wayne County and the State of Michigan. The ability to work effectively in a fast-paced environment and adapt to changing priorities is essential.
