Key Responsibilities:
- Monitor and assess the medical and health needs of individuals, ensuring timely and effective interventions.
- Visit assigned homes weekly to provide nursing oversight and complete reviews of medical services.
- Collaborate with primary care physicians, specialists, and other healthcare professionals to ensure comprehensive care.
- Advocate for individuals' healthcare needs and provide support to residential staff in maintaining health services.
- Document and maintain accurate records of medical services, including lab results, x-rays, and consultations.
- Respond to emergency requests and provide necessary documentation within established time frames.
- Provide individual-specific nursing interventions and respond to staff questions regarding medical and nursing issues.
- Complete annual reviews of medical sections on Individualized Service Plans (ISPs) and medication audits.
- Provide training and consultation to day program, IES, and residential staff on health-related topics.
- Maintain professional skills through attendance at nursing discipline meetings, online training, and discipline-specific training.
Requirements:
- Current registration as a registered professional nurse in New York State.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
