We are seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a diverse population of patients. Key Responsibilities- Patient-Centered Quality and Safety: Provide evidence-based care to patients, including accurate assessment, diagnosis, treatment, and management of health problems.
- Communication and Interdisciplinary Collaboration: Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centered care, and communicate effectively with patients, families, and healthcare professionals.
- Healthcare Environment Management: Manage clinical and non-clinical duties efficiently, adapt to new models of patient care, and enhance operational effectiveness.
- Education: Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- Experience: A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
- Skills and Abilities: Self-motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice; effective verbal, written, and electronic communication skills; outstanding organizational skills and ability to multi-task.
