About the Role
We are seeking a compassionate and skilled RN Case Manager to join our Hospice team. As a RN Case Manager, you will play a vital role in providing high-quality care to our patients and their families.
Responsibilities
- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Administer medication and treatment as prescribed by the physician, and provide education to patients and families on their care.
- Document patient progress and maintain accurate clinical notes.
- Participate in interdisciplinary group meetings to discuss patient care and address any concerns.
Qualifications
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- 1 year of nursing experience, with 3+ years preferred.
- Experience in hospice or a similar setting is preferred.
