Clinical Specialist

About the Role:
The Clinical Specialist is a vital member of our team, providing professional nursing practice for hospice care. This role involves processing workflow to ensure timely patient care and documenting compliance with policies and procedures.
Key Responsibilities:
- Provides clinical expertise in patient care, documentation, and care planning
- Processes start of care workflow and maintains orders in HCHB
- Participates in IDG and reviews visits plotted on SOC or Re-cert visit
- Manages admissions, deaths, discharges, and assists with data collection for compliance and quality assurance
- Upholds effective communication with referral sources for smooth admissions
Requirements:
- Associate degree in Nursing with current license
- Three years practice as a Registered Nurse
- Previous Hospice experience preferred
- Knowledge of the Hospice Philosophy of Care
- Ability to use Microsoft Word, Excel, Outlook, and other software programs
