Key Responsibilities
The Enhanced Care Management Registered Nurse Care Team Leader (ECM RNCTM) plays a pivotal role at AHF by integrating our Mission and Core Values with care coordination strategies. The RNCTM exhibits exceptional clinical expertise, proactive problem-solving abilities, and a thorough understanding of healthcare regulations and support services, particularly focusing on HIV/AIDS patients. This ensures that all members receive a tailored array of services that align with their individual needs and preferences throughout their care journey.
The RNCTM enhances AHF's reputation by applying fundamental nursing principles to engage with members, recognize their strengths and challenges, and assist in establishing achievable health goals to enhance health outcomes, equity, and overall quality of life.
As a passionate advocate for our primary initiatives, it is essential for the RNCTM to keep AHF's Core Values and Mission at the forefront of all activities. AHF's dynamic environment requires adaptability to provide the best possible experience for our clients and create a supportive workplace for our employees.
AHF promotes a collaborative organizational structure where staff members are accountable to multiple leaders. The RNCTM will collaborate closely with a dedicated team to foster and facilitate care coordination for health plan members, which includes developing personalized care plans, addressing care gaps, and engaging in opportunities to enhance understanding of government/grant-funded programs, health plan benefits, and community resources to ensure our members have access to necessary care.
Required Licenses and Certifications: CA RN License; BSN preferred
Preferred Experience: Acute care, Care Management, Managed Care
Bilingual in Spanish is a plus
