Job Overview

As a Health Risk Assessment Provider with i4 Search Group Healthcare, you will play a critical role in assessing and documenting high-risk conditions for patients in their homes.

This position requires a strong understanding of health risk assessment principles and the ability to identify active and chronic disease conditions, as well as physical, mental, and social needs.

Key Responsibilities:

  • Conduct health risk assessments on insured plan patients in their homes or designated areas
  • Identify and document all active and chronic disease conditions, as well as physical, mental, and social needs
  • Obtain vital signs, measure BMI, review pharmacological therapy, and conduct physical examinations
  • Conduct necessary screenings to capture HEDIS measures and close gaps in care
  • Educate and counsel patients and families on identified conditions

Requirements:

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) state license and board certifications
  • At least 1 year of experience preferred
  • Risk assessment coding experience/knowledge preferred
  • Valid state driver's license and automobile insurance
  • Ability to travel within designated counties
  • Strong computer skills, including Word, Excel, EMRs, and web-based applications
