Job Overview

Nsight Health is revolutionizing remote care solutions to help patients live longer, healthier lives.

We are seeking a highly motivated and experienced Customer Success Liaison to join our team. As a key contributor to our mission, you will work closely with physicians and clinical teams to deliver Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services directly onsite at the clinic.

Key Responsibilities
  • Build strong relationships with clinicians and their office staff to support patient enrollments in Nsight's RPM and CCM services.
  • Drive patient-centered care by working directly in clinics to enroll patients, organize data, and enhance communication between providers and patients.
  • Educate and engage patients about Nsight's care programs and drive participation.
  • Champion change by implementing team-based, patient-centered processes that improve chronic disease management.
  • Collaborate with care teams to ensure timely onboarding and care plan adherence.
Requirements
  • 5-10 years of experience in customer success, account management, or client services, preferably in healthcare.
  • A track record of driving patient enrollments and success in a fast-paced, client-facing role.
  • Strong communication and relationship-building skills, with the ability to educate and inspire.
  • Experience with HubSpot, EHR/EMR systems, and customer service tools a plus.
  • Bilingual in English and Spanish is a plus.
