DaVita Clinical Coordinator RN Job Description

DaVita is seeking a skilled Clinical Coordinator RN to join our team in the Hemodialysis Department at Northern Westchester Hospital. As a Clinical Coordinator RN, you will play a vital role in delivering high-quality patient care and leading a team of healthcare professionals.

Key Responsibilities:
  • Lead a team of healthcare professionals to provide exceptional patient care and support.
  • Develop and implement effective care plans to meet the unique needs of patients with end-stage renal disease.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and training to patients and their families on dialysis treatment and self-care.
  • Conduct regular quality assurance and performance improvement activities to ensure high-quality care.
Requirements:
  • Current Registered Nurse (RN) license in the state of New York.
  • At least 18 months of registered nursing experience, preferably in dialysis or critical care.
  • Strong leadership and communication skills, with the ability to motivate and inspire a team.
  • Excellent clinical skills, with the ability to assess patient needs and develop effective care plans.
  • Basic computer skills and proficiency in MS Word and Outlook.
