Old Colony Elder Services (OCES) is seeking a skilled Behavioral Health Clinician to join our team. As a key member of our Behavioral Health Team, you will play a vital role in supporting older adults and individuals with disabilities in the community.
Key Responsibilities- Provide direct support to older adults with complex behavioral health conditions, focusing on long-term case management and psychotherapy to ensure safe community living.
- Conduct home visits to assess and develop comprehensive care plans with consumers, caregivers, and families.
- Offer advocacy, support, and assistance to address unmet needs, including housing, hoarding, substance abuse, medical case management, and communication with providers.
- Engage in outreach to local providers about Behavioral Health services.
- Coordinate and/or facilitate support groups based on community need.
- Master's degree in social work or a related field, with current LCSW/LICSW licensure.
- Three years' clinical/community-based experience.
- Extensive knowledge of the psychological, emotional, and social needs of elders and people living with disabilities.
- Excellent assessment and communication skills.
- Strong ability to work independently.
- Bilingual skills, with Haitian Creole proficiency highly preferred for at least one position.
- Proficiency with Microsoft 365 and other software.
- A collaborative and enthusiastic team environment.
