Job Requirements

To be successful in this role, you will need to have a strong background in mental health and substance abuse treatment, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.

About the Position

This is a full-time position that requires working with adults who have mental health or substance use disorders. As a Case Manager, you will work closely with consumers, their families, and healthcare providers to develop and implement personalized service plans.

Responsibilities

  • Conduct thorough assessments to identify consumer needs and develop comprehensive service plans
  • Provide psycho-education and support to consumers and their families
  • Collaborate with healthcare providers and community resources to ensure seamless care coordination
  • Monitor consumer progress and adjust service plans as needed
  • Document all interactions and services provided in accordance with company policies and regulations

Requirements

  • Bachelor's degree in a human services field or related field
  • At least 1 year of experience working with adults who have mental health or substance use disorders
  • Valid driver's license and reliable transportation
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Compensation and Benefits

The salary for this position is estimated to be around $55,000 - $75,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package, including health insurance, dental insurance, vision insurance, and paid time off.
