We are seeking a skilled and compassionate House Calls Nurse Practitioner to join our team at Abode Care Partners.

Job Summary

This role involves providing evidence-based medical care to seniors and patients with complex needs in their homes or community settings. As a House Calls Nurse Practitioner, you will utilize your nursing expertise to examine and treat chronic and urgent health issues while collaborating with interdisciplinary teams for comprehensive patient care.

Responsibilities
  • Provide Medical Care: Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Evaluate Patient Needs: Perform evidence-based medical care by planning, directing, and evaluating patient medical needs.
  • Collaborate with Healthcare Professionals: Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
  • Order and Interpret Lab Tests: Order and interpret laboratory and diagnostic testing as needed.
  • Ensure Accurate Records: Ensure patient medical records are kept up to date.
Requirements
  • Licensure: Active state Nurse Practitioners license.
  • Certification: Current Nurse Practitioner National Board Certification.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, Strong knowledge base of HIPAA and OSHA regulations, Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
