Job OverviewThis full-time role is part of our team that provides high-quality healthcare services to underserved populations. As a Primary Care Nurse Practitioner with YesCare, you will be responsible for delivering exceptional patient care, adhering to evidence-based medical practices, and working collaboratively with our team to ensure the best possible outcomes.
Responsibilities:- Patient Care: Provide routine community standard care based on evidenced-based medicine, including sick call, chronic care, history and physicals, and emergency care needs.
- Clinical Services: Utilize available resources to treat identified problems before referring off-site. Respond appropriately in urgent or emergency situations, providing emergency treatment on-site as needed.
- Collaboration: Work effectively with multidisciplinary teams to ensure seamless delivery of care, supporting standards of medical practice through adherence to existing policies and procedures.
- Professional Development: Participate in regular meetings, attend in-service education sessions, and engage in quality improvement initiatives to continuously improve patient care and service excellence.

Requirements:- Certifications: Current certification in the scope of practice as a Nurse Practitioner (NP/ARNP).
- Licensure: Valid license to practice as a nurse in the state where the facility is located.
- Education: Graduate of an accredited Nurse Practitioner program.
- Skills: Ability to perform responsibilities as defined by state's practice act, maintain current CPR certification, and demonstrate excellent communication and teamwork skills.
