Wound Care Advanced Practice Provider

The Advanced Practice Provider (APP) will engage in a collaborative practice with physicians and other healthcare professionals in both outpatient and inpatient environments. The APP will evaluate, plan, and deliver exceptional patient care, emphasizing the diagnosis and management of prevalent medical conditions and chronic issues associated with wound care. This position necessitates advanced education, clinical training, and certification as either an Advanced Practice Nurse (APN) or Physician Assistant (PA).
Key Responsibilities
· Patient Care:
Deliver comprehensive medical services under the guidance of a supervising physician, in accordance with state regulations.
· Clinical Evaluation:
Perform detailed physical examinations and preventive health evaluations following established protocols and physician directives.
· Diagnostic Procedures:
Order, interpret, and assess diagnostic tests to identify clinical issues and determine patient care requirements.
· Care Coordination:
Document clinical findings, formulate a care strategy, and discuss these findings with the healthcare team.
· Interprofessional Collaboration:
Consult with physicians and other healthcare providers to review patient cases and care strategies.
· Practice Scope:
Function within the defined scope of practice based on education, training, and established guidelines.
· Prescriptive Authority:
Utilize prescriptive privileges in compliance with state nursing and pharmacy boards, excluding Schedule I or II substances.
· Policy Management:
Review and revise clinic policies annually in partnership with the Medical Director.
· Compliance Assurance:
Maintain precise patient documentation in accordance with federal, state, and institutional regulations.
· Program Enhancement:
Lead and engage in initiatives aimed at program development to improve patient outcomes and fulfill organizational objectives.
· Departmental Collaboration:
Collaborate with various departments to enhance the efficiency and effectiveness of wound care services.
Qualifications
· Education:
Completion of an accredited Nurse Practitioner or Physician Assistant program.
· Certification:
Current certification as an Advanced Practice Nurse (APN) or Physician Assistant (PA) in the state.
· Experience:
Prior experience in wound care or a related clinical area is preferred.
· Skills:
Strong clinical assessment capabilities, ability to collaborate effectively in a multidisciplinary setting, and excellent communication skills.
· ACLS and BLS Certification Required
