Job Summary:

The Social Work Care Manager LSW 1 is a key member of the Care Management team at Rush University Medical Center. This role is responsible for providing comprehensive psychosocial assessments, developing and implementing person-centered care plans, and coordinating transitions of care to ensure seamless patient flow. The ideal candidate will have a Master's degree in Social Work, current licensure as a Social Worker in Illinois, and experience in care management, discharge planning, and social services.

Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and families to identify needs and develop care plans.
• Develop and implement person-centered care plans that address the physical, emotional, and social needs of patients and families.
• Coordinate transitions of care to ensure seamless patient flow, including discharge planning, referrals, and follow-up care.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care.
• Provide education and support to patients and families to promote self-care and independence.
• Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

• Master's degree in Social Work from an accredited university.
• Current licensure as a Social Worker in Illinois.
• Experience in care management, discharge planning, and social services.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Strong analytical and problem-solving skills.

Preferred Qualifications:

• LCSW or commitment to obtain.
• Experience working in a hospital or healthcare setting.
• Knowledge of electronic medical records and other healthcare software.

Physical Demands:

• Ability to travel throughout the Medical Center.
