About Us:
Established in 1982, Lightways Hospice and Serious Illness Care is a dedicated nonprofit healthcare organization serving multiple counties in Illinois. Our mission is to deliver outstanding serious illness care, hospice services, and grief support to both adults and children.

Role Summary:
The Pediatric Social Worker plays a crucial role in providing immediate crisis intervention, along with individual and family counseling. This support is tailored based on the social worker's evaluation, family feedback, and the interdisciplinary team (IDT) care plan.

Key Responsibilities:
1. Develop and nurture relationships with referral sources.
2. Offer relevant information and support to patients and their families or caregivers as needed.
3. Conduct field visits to patients' residences, nursing homes, or other living environments.

Work Environment:
This position requires travel within the designated territory, which extends to the Wisconsin border. The Pediatric Social Worker will need to attend office meetings twice a month.

Qualifications:
Applicants must possess a Master of Social Work (MSW) degree and hold a Licensed Social Worker (LSW) certification. A minimum of one year of experience working with pediatric patients is essential.
