Job Overview

We are seeking a skilled Home Health Physical Therapist to join our team. As a leading provider of home health care services, we offer a unique opportunity to make a meaningful difference in the lives of our clients.

Estimated Salary Range

The estimated salary range for this position is $ $90,000 - $110,000 per year, depending on experience and qualifications.

About the Role

This full-time opportunity involves providing high-quality physical therapy services to adult and geriatric clients in their homes. You will be an integral part of a multidisciplinary healthcare team that delivers rehabilitative care and skilled nursing to our clients, allowing them to receive the medical attention they need to remain at home.

  • Perform diagnostic tests and measurements to assess client needs and develop personalized care plans.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, balance, and other therapies as needed.
  • Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary to promote independent living.
  • Document observations, interventions, and evaluations accurately using state-of-the-art technology.
Requirements

To be successful in this role, you will need:

  • A current STATE Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
