We are seeking a highly skilled Home Health Physical Therapist to join our team at LHC Group in Egg Harbor Township, New Jersey.

About the Role

This is an exciting opportunity to work with patients who require ongoing physical therapy services in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing individualized treatment plans, and delivering high-quality physical therapy services.

Key Responsibilities
  • Evaluate patient functional status, medical conditions, and physical therapy needs to develop personalized treatment plans.
  • Provide expert physical therapy services, including pain management, mobility enhancement, and functional restoration, to patients in their homes.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure comprehensive patient care.
  • Document patient progress, outcomes, and any changes in condition, adhering to regulatory requirements and agency policies.
  • Participate in staff conferences, committees, and educational programs to stay current with industry developments and best practices.
Requirements and Qualifications
  • Current licensure as a Physical Therapist in the state of New Jersey.
  • Current CPR certification.
  • A valid driver's license and reliable transportation.
  • Ability to work independently, manage multiple patients, and prioritize tasks effectively.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse healthcare professionals and patients.
