About the Role

River Edge Behavioral Health is seeking a skilled Hospital Discharge Liaison to facilitate the reintegration of clients from an inpatient environment back to the community.

Key Responsibilities
  • Build and maintain relationships with discharge authorities to ensure seamless transitions.
  • Coordinate all appropriate procedures in partnership with interdisciplinary treatment teams and other agencies.
  • Develop and implement effective discharge plans to ensure continuity of care.
  • Assess patients' needs, identify potential barriers, and develop strategies to overcome them.
  • Manage multiple cases and deadlines simultaneously, utilizing exceptional organizational and time management skills.
Requirements
  • Master's degree in Social Work, Sociology, Criminal Justice, or Psychology.
  • Candidates eligible for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, and LAPC are preferred.
  • Strong problem-solving and critical thinking abilities.
  • Exceptional communication and interpersonal skills.
