Job Summary

We're seeking a skilled Registered Nurse with a focus on community health education and experience to join our clinical/field team. As a member of our team, you'll be responsible for planning, organizing, and directing home care services that meet the unique needs of our clients.

Key Responsibilities

  • Plan and direct home care services to meet the needs of individuals and families
  • Build relationships with community resources to enhance care delivery
  • Collaborate with interdisciplinary teams to ensure comprehensive care

Requirements

  • Current and unencumbered RN licensure in the state of employment
  • Bachelor's degree in Nursing from an accredited program (preferred)
  • 1-2 years of recent acute care experience in an institutional setting
  • Preferred: 1 year of home health care experience and management/supervisory experience
