Patient Advocate Representative

As a Patient Advocate Representative with Conifer Revenue Cycle Solutions, you will play a critical role in ensuring that patients receive the necessary support and resources to navigate the healthcare system. Your primary responsibility will be to screen self-pay patients at hospital bedside for eligibility in various governmental and non-governmental programs. You will also be responsible for identifying all sources of potential payors, including auto insurance, Workers' Compensation, commercial insurance, private insurance, and TPL, to route accounts appropriately in the Patient Accounting environment.
Key Responsibilities- Conduct interviews with patients and/or family members to gather necessary information and assess their eligibility for various programs.
- Record and maintain complete documentation of activities performed on account while in-house and during the Patient accounting cycle.
- Perform financial clearance functions, including collections, and cancel accounts that have not had any patient cooperation and are not eligible for any programs.
- Follow up on EES assigned accounts to ensure follow-through on Government application submitted and develop a working relationship with patients based on good communication skills.
- Conduct field visits to patient homes for skip tracing and/or assisting patients with documents.
- Notify hospital case management, social services, and admissions staff of case screening determinations and outcomes via verbal and written communication.
- Working familiarity with the rules and regulations pertaining to Federal, State, and County programs.
- P/C systems literate, including Windows, and Microsoft Outlook, Excel, and Word programs.
- Ability to work independently and prioritize multiple tasks with efficiency.
- Excellent oral and written communication skills, as well as a clear understanding of the English language.
- Detail-oriented, with strengths in dealing with multiple facilities, supervisors, and hospital platforms.
- Ability to travel if required.
- High School diploma or equivalent.
- Minimum 2 years of work experience in Social Services or Hospital Admitting or related area.
