Job Summary

We are seeking a skilled LPN Case Manager to join our team at Memorial Healthcare System. As a key member of our healthcare team, you will be responsible for developing and implementing patient care plans, monitoring patient progress, and collaborating with healthcare providers to ensure high-quality patient outcomes.

Key Responsibilities:

  • Develop and implement patient care plans based on individual patient needs and goals
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with healthcare providers to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
  • Maintain accurate and up-to-date patient records

Requirements:

  • Graduate of an accredited Practical or Vocational Nursing program
  • Current LPN licensure in the State of Florida
  • One year of experience as an LPN in a healthcare setting
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks

Working Conditions:

This position requires the ability to work in a healthcare setting, which may involve exposure to infectious diseases and other health risks. The successful candidate will be required to undergo a background check and provide proof of current LPN licensure.
