Key Responsibilities
  • Provide skilled nursing care and treatment to clients in their homes, as per physician orders and care plans.
  • Accurately document nursing actions, client health status, and communicate with family members and physicians.
  • Develop and implement individualized care plans, focusing on health promotion, disease prevention, and management.
  • Conduct regular nursing assessments, identify potential health risks, and develop strategies to mitigate them.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Administer medications, manage medication regimens, and educate clients on proper medication use.
  • Provide health education and training to clients and their families, promoting self-care and independence.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices.

Requirements
  • Degree in Nursing from an accredited institution.
  • Current and unrestricted license to practice nursing in the state of operation.
  • One or more years of nursing experience, preferably in home health care or a related field.
  • Basic clinical skills, including medication administration and management.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a team, with a keen attention to detail and a commitment to quality care.
