Job Summary

Care Alliance Health Center is seeking a skilled LPN to join our team as a Community Health Outreach Specialist. In this role, you will collaborate with our healthcare team to provide supportive medical care to clients in need, regardless of their ability to pay.

Key Responsibilities
  • Work with our outreach and primary care clinics to provide nursing care in a mobile clinic or community health ambulatory care setting.
  • Support providers in the examination and treatment of patients, including preparation of exam rooms and instrumentation, and operation of medical equipment.
  • Perform triage/intake on all patients according to Care Alliance triage/intake procedures to expedite Provider's daily schedule and patient flow.
  • Engage and educate the community on services provided by CAHC.
  • Room patients and establish reason for visit, documenting patients' chief complaint and completing all EPIC visit encounter fields and initiating visit note for provider.
  • Review medication lists, allergies, pharmacy, and immunizations.
  • Complete blood draws, lab work, and assist with women's health vaginal exams.
  • Under the supervision of the Clinical Outreach provider, perform basic health assessments, vaccinations, injectables, and procedures.
  • Properly dispose of packages, containers, materials, and biohazards, as well as expired/wasted medications in appropriate containers.
  • Perform basic health screenings and report pertinent findings to management or provider.
  • Implement appropriate components of the plan of care, under the direction of the healthcare team, ensuring all needed POCT are completed and documented in EMR prior to provider visit.
  • Familiarize yourself with sterile techniques and setting up of a sterile field.
  • Work collaboratively with other Care Alliance health professionals to facilitate a team approach to the management of patients.
  • Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures, complying with legal, HRSA, and OSHA regulations in collaboration with the healthcare team.
Requirements
  • High School diploma or equivalent and completion of a state-approved nursing program.
  • Current license to practice as an LPN in the State of Ohio.
  • Minimum of 2 years' experience as a LPN in an outreach environment.
  • Comfortable working with patients in various settings, including people who are in outreach clinics, shelters, encampments, and homeless.
  • Good problem-solving, computer, verbal, and written communication skills.
  • Ability to efficiently use EPIC.
  • Current BLS Certification.
